Appointment Request

Instructions for new clients:

  • To request an appointment or to obtain additional information about counseling services, please fill out the form below. 
  • Your request for an appointment will generate a secure message to the email address you provided, which will prompt you to create a login to the secure portal. 
  • Once you create a login, you will have access to forms that will need to be completed prior to your first appointment. 
  • Once you have created a login on the secure portal, you will have the option to select a date and time for your initial appointment, based on availability. 
  • Please note, we are in-network with the following insurance providers: Anthem, Blue Cross + Blue Shield, United Healthcare, Cigna/Evernorth, and Optima. If we are not in-network with your insurance provider, you will be billed our usual and customary rate for services. We are able to provide. documentation for you to file an out-of-network claim with your insurance provider, although Willowbrook Counseling cannot guarantee that your insurance carrier will reimburse you for our services.
